			<content:encoded><![CDATA[<p></p><p style="text-align: center;"><strong>How to Stop Neck Pain, Back Pain, and Sciatica&#8230;Forever!</strong></p>
<p><em><strong>A funny thing happened on the way to my demise&#8230;I reclaimed my life!</strong></em></p>
<p>Am I being melodramatic? Not at all! In fact, at the time I was trying to figure out just how my family was going to make it in my absence. I was in really bad shape! I had lost 10 years of my life to health issues, some I don&#8217;t even want to think about any more, most centered around chronic and acute neck pain, back pain, and sciatica. </p>
<p>Suffice it to say, I was getting ready to call it quits, one way or the other. </p>
<p>You see, I had reached the point where I had to decide what would be next, whether I would give life one last try or punt. I had to decide whether I was going to let hopelessness and ill health take my life or fight back. I was battling chronic neck pain, back pain, sciatica (most of my spine was involved); Type II Diabetes (insulin dependent and oral medication four times a day); high blood pressure; congestive heart failure; and the real killers, a combination of depression and addiction to the pain medication, pain meds that had been my life for over ten years. I was a legal addict dealing with a &#8220;controlled addiction!&#8221; </p>
<p>Pain medication for the chronic and acute neck pain, back pain, and sciatica was prescribed each and every month in massive quantities (<em>e.g.</em>, Oxycontin, Fentanyl, and a number of others); and, there was nothing controlled about the addiction&#8230;<em>nothing!</em></p>
<p><em><strong>Like I said, a funny thing happened on the way to my demise! </strong></em></p>
<p>Not so funny really, not when you think about it. Perhaps ironic? Perhaps it was just sad? Yes, sad! Sad in the sense that I had wasted so much of my life. Sad because I had believed the doctors, nurses, hospitals, physical and occupational therapists, psychologists, and psychiatrists who told me there wasn&#8217;t as lot they could do for me, that the situation was too complex, that there was little hope of any sort of a recovery. </p>
<p><em><strong>Fooled them!</strong></em></p>
<p>One doctor said: &#8220;<em>If you had no arms or legs, you would accept the fact that you are never going to walk again, that you would be forever disabled. Well, you are actually in the same situation. So, accept the cards you have been dealt and make the best of it!</em>&#8221; He walked out of the examination room and I walked out of his practice&#8230;<em>for good!</em></p>
<p>Another doctor said: &#8220;Your case is just too complex. Frankly, I wouldn&#8217;t know where to begin. Honestly, I would rather not take you on as a client.&#8221;</p>
<p><em><strong>That one took a while to get over!</strong></em></p>
<p>Still another specialist, the guy who ended up saving my life, called me &#8220;s<em>alvagable!</em>&#8221; My wife cried.</p>
<p><em><strong>&#8220;Salvagable!&#8221;</strong></em></p>
<p>I will never forget that word. I felt like human refuse! But at least he was offering something approaching hope. </p>
<p>The interestingly part of it all? That conversation actually presaged the most difficult part of my journey back because it was in that instant I learned I still had a chance. It&#8217;s a funny thing about hope, it cuts both ways. Sometimes it&#8217;s easier to resign yourself to your fate than it is to fight back. </p>
<p><strong>Well, after several back surgeries, a myriad of treatments, and days, weeks, months, even years in the hospital, I triumphed! </strong></p>
<p>The road took many turns and it was a terrible journey. At times, the road back was so difficult I wanted to quit, really wanted to just quit! I ended up stuck in a dead end or two along the way and the biggest hurdles, mentioned above, were depression and addiction. The combination of depression and addiction was more difficult to overcome than any of the physical ailments. Combined with osteomyelitis and cancer, the depression and addiction meant that every day was a gauntlet, pummeled from all sides simultaneously. Interestingly, the psychological pain, and the ordeal of fighting with and through a terrible addiction, was every bit as horrible as the physical pain. At times, the psychic pain would take me to the depths of despair; and, at other times, it was the chronic and chronically acute neck, back, and sciatic nerve pain that would do it. When all guns were firing simultaneously? It was as if I were in hell on Earth!</p>
<p>Ultimately, I triumphed over the pain, psychic and physical. In retrospect, I don&#8217;t know how I did it, how I made it through, but I did! I learned a great many things about myself and a number of things about others, particularly about those closest to me. Significantly, I also learned many things about the medical community; and, many were not very complimentary. </p>
<p>What I learned about myself? I can take more pain and go through more hardship than I would ever have imagined. Of those close to me? That they love me and that without them I would have been doomed. About the medical community? They do not like to admit it when they don&#8217;t have the answers; and, they are unwilling to acknowledge that fact to patients, even if it may cost the patient his or her life. Additionally, there are good doctors and bad, just like there are good mechanics and bad, good accountants and bad, good dog groomers and bad. The difference? An incompetent physician, one unwilling and/or unable to admit his or her shortcomings may cost you your life! </p>
<p>The moral of the story? If you can call it that? If you are suffering from chronic and/or acute pain, get a second opinion. If you don&#8217;t agree with your physician, get a second opinion. If the quality of your life has been affected, get a second opinion. And, if a physician wants to put you on narcotics for chronic neck pain, back pain, and/or sciatica treatment, run screaming from the office and, get a second opinion!</p>

<p><em><strong>As we progress through our neck pain, back pain, and sciatica relief exercise program, the way we deal our condition in terms of stretching and exercise will evolve and grow more targeted and intense. As our program changes, the need for the proper program and workout facility will change, as well!</strong></em> </p>
<p>The weather is changing here in the Midwest, it is growing cold and wet! However, even for folks Downunder and in other regions of the World south of the equator, facing the prospect of Summer, the next step in a complete neck pain, back pain, and sciatica relief exercise program is choosing the right gym. Choosing a gym is second only in importance to chosing your medical practitioner; and, followed closely by choosing a personal trainer, to be covered in a subsequent article. </p>
<p>Choosing a medical practitioner is the single most important decision you will make as you deal with your chronic neck, back, and/or sciatic nerve pain issues. I would argue that choosing the right gym is even more important than selecting a physical therapist whom, after all, is usually someone you are referred to and see only as long as the insurance stays in effect.</p>
<p>Once your medical practitioner has cleared you for a stretching and exercise program, and you have finished the generally short stint with your physical therapist, it is time to chose a gym.</p>
<p>As a neck pain, back pain, and sciatica sufferer, you will require certain considerations a &#8220;normal&#8221; gym members will not and it is crucial you interview the gym thoroughly and that you make certain your personality and that of the gym are in sync. Gyms, like people, have very distinct personalities and it is crucial to the success of your long term neck pain, back pain, and sciatic nerve pain (sciatica) relief exercise program&#8217;s success that they match is a good one. </p>
<p><strong>The factors to consider as you consider various gyms and health and fitness centers, those conducive to your neck, back, and sciatic nerve pain issues, are:</strong></p>
<p>1)  Location<br />
2)  Cleanliness<br />
3)  Staff, including the availability of a qualified and competent instructor familiar with neck pain, back pain, and sciatica exercises and, if possible, someone who has first hand experience dealing with the effects of chronic neck pain, back pain, and sciatica<br />
4) The right balance of equipment (<em>i.e.</em>, machines, bikes, other types of cardio machines, free weights, etc.)<br />
5) Music and overall socio-cultural personality<br />
6)  Locker rooms<br />
7)  Shower and restroom facilities (and their cleanliness-a key consideration)<br />

<p style="text-align: center;"><strong>Are You a Victim of Neck Pain, Back Pain, and Sciatica Diagnosis Bias?</strong></p>
<p><em><strong>Diagnosis bias is a real issue in the medical community, one not often discussed, openly or in private. In fact, diagnosis bias is one of those &#8221;dirty little secrets&#8221; only recently addressed; and then, only in closed door conferences and at a few of the more progressive medical schools in the country.</strong></em></p>
<p>Chronic fatigue and chronic to chronically acute neck pain, back pain, and sciatica are among the most difficult conditions to diagnose and treat. A great deal of the reason, or reasons, for the difficulty in properly diagnosing and treating chronic pain and chronic fatigue, to include chronic neck pain, back pain, and sciatica is because while pain is a very real symptom of an underlying condition, and the underlying condition may be recognized and treatment initiated, it is also an abstract concept. </p>
<p><strong>Pain as an Abstract Concept: Difficulties in Diagnosis and Treatment</strong></p>
<p>When I state that pain is an abstract concept, I mean pain in any of its guises, in any and all of its manifestations, is as different from one person to the next as personality and/or fingerprints are. For this reason, and a myriad of others, neck pain, back pain, and sciatica create a number of challenges for medical practitioners, not only in the diagnosis phase but in subsequent treatment.</p>
<p><strong>Value Attribution, Diagnosis Bias, Neck Pain, Back Pain, and Sciatic Nerve Pain (or Sciatica)</strong></p>
<p>When dealing with neck pain, back pain, and sciatic nerve pain (sciatica) complaints doctors, like most people, will often make an initial assessment based on value attribution. Value attribution, in this instance at least, is a <em>first impression of the medical kind!</em> Unfortunately for the individual suffering from chronic fatigue or chronic neck pain, back pain, and/or sciatica the medical practitioner will often stick to that attribution when making even a critical diagnosis, even in the face of contradictory evidence. </p>
<p>Value attribution and diagnosis bias occur for a myriad of reasons, most of all because doctors are human. Medical practitioners are subject and susceptible to the same biases and prejudices the rest of us are. Interestingly, this bias, once set in place, will often shape the medical practitioner&#8217;s future perception of a patient&#8217;s complaints, once again, even when confronted with contradictory evidence, and will shape the treatment received&#8230;or not received.</p>
<p>Value attribution and diagnosis bias are covered extensively, and quite well, by Brafman and Brafman in their book <em><a href="http://www.amazon.com/gp/product/0385530609?ie=UTF8&#038;tag=wwwultimateso-20&#038;linkCode=as2&#038;camp=1789&#038;creative=9325&#038;creativeASIN=0385530609">Sway: The Irresistible Pull of Irrational Behavior</a></em>. I recommend it for anyone who would like to understand why we do the things we do and make the decisions me make&#8230;and are forced to live with. This book is particularly appropriate for anyone who feels he or she may have been a victim of diagnosis bias. </p>
<p><strong>The Video, Chronically Acute Back Pain, and Diagnosis Bias</strong></p>
<p>I have first hand knowledge of diagnosis bias as it applies to neck pain, back pain, and sciatica and I discuss the situation and its eventual outcome in the video below. It should be an eye-opener for many, for others living through it at this time, it may give you what you need to press on and fight back, fight against the diagnosis bias that nearly took my life and ultimately robs many neck pain, back pain, and sciatica sufferers every year of the life they could have if only someone took them seriously&#8230;or just took them differently!</p>
<p><strong>Chronic Fatigue, Chronic Pain, Empathy, and Abstraction</strong></p>
<p>In fighting chronic fatigue, and chronic pain, one very often goes through the same battles to be heard and taken seriously. Many times even friends and relatives just don&#8217;t understand. How could they, pain is such an abstraction and if they have never experienced it themselves? Again, how could they! </p>
<p>Given the above, one would still hope the medical community, and particularly specialists trained to deal with chronic fatigue and chronic neck pain, back pain, and/or sciatica would be somewhat immune from value attribution and diagnosis bias. Yet we know that, in fact, some are and some aren&#8217;t! </p>
<p><strong>The Good, The Bad, and The Misdiagnosis</strong></p>
<p>Just as there are good physicians and bad ones, there are good chefs and terrible ones, there are good pilots and bad ones&#8230;and they all make mistakes. The trick here is to not allow others to lump you into a category before an adequate diagnosis has been made. Whether the condition is a chronic one, one that is chronic and acute or<br />
is relatively new and still very much in the acute phase, do not allow your medical pracitioner to make an attribution, a diagnosis, based on a bias and without all the facts. As stated above, pain in any and all of its guises is a symptom and it is their task to uncover the cause and treat it effectively. </p>
<p><strong>Do Not Take No for an Answer!</strong></p>
<p>And do not take &#8220;I don&#8217;t know&#8221; for an answer! Press on until someone, somewhere provides you with a legitimate diagnosis and the quality care you deserve, care we all deserve. There is a life on the other side of any chronic fatigue and/or pain condition, do not allow anyone to rob you of even one day of your life&#8230;much less your life in total, as almost happened to me!</p>

<p><em><strong>Chronic pain generally, and neck pain, back pain, and sciatica specifically, have been historically difficult to manage. Relief from chronic and/or acute pain with the use of narcotics is coming under increasing scrutiny as addiction becomes common and the efficacy of narcotics as a pain relief strategy is hotly debated. </strong></em></p>
<p>The fact is, narcotics are not recommended as a neck pain, back pain, and sciatica relief strategy. In ever increasing numbers, pain management clinics are coming under fire for prescribing massive amounts of narcotics with little or no demonstrable reduction in the level of pain for their clients. Many pain management clinics have been closed down, either by litigation, the threat of litigation, or by state and federal government agencies. Still others have been forced to rethink and reshape their approach to pain management in order to survive.</p>
<p>Many pain management patients have become addicted to narcotics and have had to undergo difficult and protracted drug rehabilitation therapy once, twice, even three and four times, just to get &#8220;clean.&#8221; Pain management clinics have been sued by their former patients and many pain clinics refuse to take on new patients already taking narcotics for fear of taking on yet another problem patient.</p>
<p>Neck pain, back pain, and sciatica patients stuck in this trap, with very few options available to them, are often left on their own with nowhere to turn and no physician willing to help them. If additional surgery is required, many surgeons refuse to take on this sort of patient due to the high risk nature of the relationship.</p>
<p>The video below discusses the new drug Suboxone, one that may offer new hope to tens of thousands of pain management patients currently without  hope and without a future. Suboxone has been nothing short of a miracle drug for many, contributing to neck pain, back pain, and sciatica relief, while at the same time allowing an individual to come off of the heavy narcotics in favor of a &#8220;kinder gentler&#8221; pain management strategy. </p>
<p>While still technically an opiod, Suboxone has features that make it appropriate for not only pain management, still being debated in the medical community, and as an addiction treatment, one for which the FDA regulations were considerably loosened in late 2006. Now, a generic Suboxone is available, making it more affordable for everyone. The loosening of regulations means more doctors can prescribe the drug which means many more will have access to it, thus offering hope to many people stuck in the limbo of pain management, still in pain but wishing to &#8220;come clean.&#8221;</p>
<p><strong>This is a very anecdotal, unscientific post and before considering any kind of medical treatment, you must, of course, consult your physician!</strong> </p>
<p>However, compared to the alternatives: narcotics, addiction and all; methadone, with its own form of addiction and consequences; and, neck pain, back pain, and sciatica, which we have covered extensively; there can be little doubt that Suboxone at least offers a way up and out. </p>

			<content:encoded><![CDATA[<p></p><p style="text-align: center;"><strong>Neck Pain, Back Pain, and Sciatica: Understanding and Dealing With Failed Back Surgery Syndrome</strong></p>
<p><strong>Spine surgeries are performed for a myriad of conditions and complaints every year here in the United States and around the world. Of the spine surgeries performed, an incredibly high number result in what is known as failed back surgery syndrome. </strong></p>
<p><em><strong>When referring to the terms spine surgery and back surgery, I will be applying them in their broadest sense, meaning neck, upper, middle, and lower back surgery.</strong></em> </p>
<p>Back surgery is generally the result of a number of complaints and conditions, usually all related in some way to acute and/or chronic neck pain, back pain, and/or sciatica. Failed back surgery syndrome is generally a continuation of pain, known as chronic pain, and other symptoms affecting the patient. </p>
<p><strong>The medical names for the various back pain expressions are as follows:</strong></p>
<p>1) <strong>Back pain</strong>, also known as <em><strong>dorsalgia</strong></em>, is a generalized pain experienced, mild to acute, isolated to chronic, in the back and originating as the result of a number of conditions, affecting a variety of structures (e.g., nerves, vertebrae, joints, intervertebral discs, etc). Back pain is said to affect as many as 95% of all adults at some time in their lives.</p>
<p>2) <strong>Neck pain</strong>, also known as <em><strong>cervicalgia</strong></em>, affects as many as 66% of all adults at some time in their lives. Neck pain may also express as pain in the shoulder, arm, and hand, with associated numbness and tingling into the hands and fingers.</p>
<p>3) <strong>Upper back pain</strong>, also referred to as <em><strong>thoracic pain</strong></em> or <em><strong>middle back pain</strong></em>, is expressed along the spine from C7 to L1, or from the base of the neck to the base of the thoracic region of the spine. Upper or middle back pain is often but not always isolated to a specific location and has a lower incidence than neck or lower back pain.</p>
<p>4) <strong>Lower or low back pain</strong>, also known as <em><strong>lumbago</strong></em>, is pain in the lumbar region and may be localized or may radiate laterally. In its worst form, lower back pain will be accompanied by sciatica or leg pain radiating into one or both legs.</p>
<p>5) <strong>Sciatic nerve pain or sciatica</strong>, also referred to as <em><strong>sciatic neuritis</strong></em>, often associated with <strong><em>lumbar radiculopathy</em></strong>, and commonly referred to as &#8220;<strong><em>leg pain</em></strong>,&#8221; generally arises as a result of an irritation or compression of one of the five nerve roots that ultimately make up the sciatic nerve. The leg pain may extend into the feet and often ranges from acute to chronic and sensation from tingling to numbness of the legs, feet, and even the toes.</p>
<p><strong>Stress and Trauma Induced Neck Pain, Back Pain, and Sciatica</strong></p>
<p>The pain expressions listed above, neck pain, upper and middle back pain, lower back pain, and sciatica, are complex and often arise as the result of stress placed on the spine and the reaction of the spine and the supporting musculoskeletal system to a specific stressor or constellation of stressors, whether generalized or specific. The spine may also be traumatized as the result of an isolated event, thus resulting in pain to a specific location and ranging once again from acute to chronic, depending on a myriad of factors.</p>
<p><strong>Failed Back Surgery Syndrome: Failure of Expectations</strong></p>
<p>Often, particularly when the reason for the back pain is the result of a number of factors, back surgery may not yield the anticipated result. The failure of the back surgery thus leading to a continuation and, in many instances, increasing levels of neck pain, back pain, and/or sciatica, and failed back surgery syndrome.</p>
<p><strong>Failed Back Surgery Syndrome: Odds of Success and Behaviors</strong></p>
<p>It is estimated that several million individuals see a medical practitioner daily for neck, back, and sciatic nerve pain and that between four hundred thousand and five hundred thousand individuals in the United States alone undergo back surgery in one form or another every year. Of the half a million neck pain, back pain, and/or sciatica sufferers who submit to back surgery every year, between 20% and 40% continue to have significant issues, to include similar or increased levels of pain, overall muscle weakness, excess weight and/or obesity, sleep disorders, depression, numbness and tingling of the extremities, and disability&#8230;to list just a few.</p>
<p><strong>Failed Backs and Frustration Across the Board!</strong></p>
<p>There is a general sense of frustration, not only on the part of the surgical patients and their families, but also within the medical community, as failed back surgery syndrome is one of the most perplexing issues facing orthopedic medicine today. The condition is not an isolated syndrome, and in fact to call failed back surgery syndrome a &#8220;syndrome&#8221; is a misnomer, it is a condition resulting from a constellation of issues and a myriad of factors, many completely unrelated to the surgical procedure itself.</p>
<p><strong>Failed Back Surgery Syndrome: Reinforcing Results and Behavior</strong></p>
<p>Individuals with failed back surgery syndrome, also referred to as post-laminectomy syndrome and failed spine surgery, or simply failed backs, generally have chronic to chronically acute pain affecting the original area of concern. Neck pain, back pain, and sciatica may grow worse with time and the pain may become debilitating and disabling. The resulting pain often leads to guarding, decreased activity, loss of muscle tone and sensation, over all malaise, and often addiction to pain medication.</p>
<p><strong>A Mixed Bag: Why FBBS? </strong></p>
<p>Failed back surgery syndrome may result from an ill-advised surgery, an ill-prepared surgical patient, an improperly executed surgical procedure, an incomplete surgical procedure, an incomplete or inadequate post-operative experience to include under prescribed physical therapy or incomplete physical therapy regimen, and a myriad of other reasons and consequences. Failed back surgery syndrome may also be the result of a number of psycho-social and socio-economic factors, as has been suggested in the medical literature lately as orthopedic medicine attempts to come to grips with this complex and frustrating issue. </p>
<p><strong>The Reality of Failed Back Surgery Syndrome: Get a Second Opinion!</strong></p>
<p>The reality of the matter is, regardless of cause, failed back surgery syndrome is a fact of life for tens of thousands of neck pain, back pain, and sciatica sufferers every year&#8230;and there is little hope in sight that this complex issue will be resolved any time soon. Ultimately, before agreeing to any sort of surgical intervention for neck, back or sciatic nerve pain, an individual should get a second opinion and explore every avenue of relief possible first&#8230;because the risk of a failed back surgery is real.</p>

			<content:encoded><![CDATA[<p></p><p><strong>Treating chronic neck pain, back pain, and/or sciatica is always problematic. The condition may lead to debilitating pain and, ultimately, to disability! </strong></p>
<p>Most people, at some point in their lives, experience some type of short-lived back pain. This is usually a relatively temporary occurrence that doesn’t interfere with daily life in a major way. But, for an unlucky percentage of the population, back pain is a chronic problem that’s debilitating physically and emotionally. To make matters worse, treating and managing chronic back pain tends to be a frustrating process because not all causes are totally understood. The first step to treating and managing chronic back pain is finding a doctor who specializes in back disorders, an orthopedic surgeon or a neurologist. This doctor will be able to diagnose what type of pain you have and, hopefully, determine what is causing it. </p>
<p>Neck pain, back pain, and/or sciatica may be muscular or nerve-related. It may occur in the upper or lower portion of the back. (Lower back pain is the most common type, and some studies indicate that its prevalence is on the rise, possibly due to a higher percentage of the population being overweight or obese.) It may be the result of an injury or it may be age-related. Common causes of chronic back pain include herniated discs, arthritis and sciatica. </p>
<p>Conventional treatments for chronic neck pain, back pain, and sciatica include oral medications, injections, weight loss and surgery. If you have tried these options without success, or if the cause of your pain has been diagnosed as ‘non-specific,’ it may be time to consider alternatives such as physical therapy, reducing stress, getting more and better sleep, changing your diet, getting acupuncture or acupressure treatments, going to a masseuse regularly, using an inversion board, or getting chiropractic adjustments.  </p>
<p><strong>Physical Therapy</strong> </p>
<p>If your back pain is caused by muscular stiffness or inflammation, physical therapy may be helpful. The purpose of physical therapy is to loosen and work muscles to improve your mobility. You may be able to independently perform flexion (bending forward), extension (bending backward) and other stretching exercises. Or, you may need to go to a specialist to help you perform the needed movements (possibility in conjunction with apply heat/cold treatments and/or electrical stimulation).</p>
<p><strong>Stress Reduction </strong></p>
<p>High levels of stress intensify the body’s sensitivity to pain. Therefore, reducing stress is one way to reduce neck pain, back pain, and sciatica. Consider visiting a mental health care specialist to develop a plan of action for reducing stress in your life. Managing stress through the use of regular meditation or deep-breathing exercises can be helpful as well. You may prefer to practice such exercises on your own, or as part of a guided group. </p>
<p><strong>Improving Sleep </strong></p>
<p>Not getting enough sleep, or having poor-quality sleep, can be both a cause and a symptom of chronic back pain. If you feel tired in addition to having back pain, or frequently wake up at night in extreme discomfort, consider spending some time addressing this aspect of your overall health. Of course, sleeping on a comfortable mattress that properly aligns your spine is important. But ruling out disorders like sleep apnea is a smart idea. No matter what, if you’re as rested as you can be, you’ll be likely to experience less pain and be better able to deal with the pain you do have.</p>
<p><strong>Dietary Changes</strong> </p>
<p>It’s probably pretty obvious that eating a healthful, varied diet and maintaining a consistent, healthy weight is vital to overall well-being. But other, more specific changes to your diet might help reduce back pain and sciatica. For example, you may have food allergies or sensitivities that you’re not aware of, deficiencies in specific nutrients or vitamins (like Vitamin D), or undiagnosed digestive problems (like celiac disease). Elimination diets, allergy testing and other diagnostics can help you determine if a diet-related problem is causing or exacerbating your back pain. </p>
<p><strong>Acupuncture and Acupressure</strong> </p>
<p>While not totally accepted in the U.S. nor confirmed to be effective, this practice is gaining popularity. It involves inserting thin needles into the skin at specific points on the body to unblock ‘Qi’ or ‘life-force’ channels. Studies have confirmed that acupuncture may be effective in reducing neck pain and back pain if combined with other treatments. Some practitioners use the same principle to perform acupressure, in which pressure (rather than needles) is applied to specific points on the body. </p>
<p><strong>Massage </strong></p>
<p>As with physical therapy, if your neck pain, back pain, and/or sciatica is caused by muscle tightness, massage may help. Massage may be general in nature, or more specialized as with ‘rolfing,’ a practice that involves loosening the fascia (tissue covering muscles) in the back through the use of strong pressure. </p>
<p><strong>Inversion Therapy </strong></p>
<p>If you have back pain and sciatica caused by a compressed disc or sciatica, inversion therapy (a form of ‘traction’ treatment) may provide short-term relief. It involves hanging upside down by the ankles or tipping upside down in a special table, which allows gravity to stretch the spine, decompressing nerve roots and discs in the process. This isn’t a long-term solution to chronic pain, but might be helpful in combination with other therapies. </p>
<p><strong>Chiropractic Treatment </strong></p>
<p>This type of treatment involves physical manipulation of the spine and/or surrounding tissues to alleviate neck pain, back pain, and/or sciatica. It is performed by chiropractors and osteopathic physicians, and may be helpful but shouldn’t be used if you have certain conditions such as compressed spinal cord or inflammatory arthritis. Check with your primary doctor first. </p>
<p><strong>In most cases, the options discussed here represent ways to manage chronic back pain rather than cure it. One or more of these options may temporarily eliminate or alleviate your pain, but it’s quite likely that you’ll need ongoing treatment to maintain a pain-free or pain-reduced life. Exercise, weight loss, and any one or several of the above strategies, when applied in concert will alleviate and possibly eliminate neck pain, back pain, and sciatica once and for all!</p>

			<content:encoded><![CDATA[<p></p><p><strong>The Myofascial Pain Syndrome or MPS is a key component in the Musculoskeletal Pain Syndrome and chronic pain expression, as is fibromyalgia, as with neck pain, back pain, hip or pelvis pain, and sciatica! Pain is localized and intense, raging from one extreme to the other, and may be debilitating and disabling.</strong></p>
<p>In order to understand the musculoskeletal pain syndrome, we must examine fibromyalgia, which we have already done. Now, we will examine the other half of this very complex pain syndrome, the myofascial pain syndrome or MPS. The myofascial pain syndrome describes and defines a condition characterized by chronic pain, often associated with neck pain and back pain, as well as sciatica. Myofascial pain syndrome is best known for pain caused by &#8220;trigger points&#8221; or TrPs. Trigger points are localized pain centers or points, at times expressed as painful knots or contractures found in any skeletal muscle, anywhere on the body. Researchers have visibly napped and identified these &#8220;knots&#8221; which may express pain as anything from referred pain to very specific and intense pain in other parts of the body. In other words, myofascial pain syndrome symptoms may vary from referred pain at various myofascial trigger point,s to specific and localized pain in other areas of the body.</p>
<p>As noted above, MPS is closely related to the complex musculoskeletal pain syndrome known as fibromyalgia. Whereas fibromyalgia pain is expressed generally and occurs above and below the waist, on the right and left sides of the body, MPS, with its associated pain syndrome, is often more localized and found in more circumscribed areas of the body. Myofascial pain is more frequently expressed around the neck and shoulders, and is usually found on only one side of the body.</p>
<p>In both myofascial pain syndrome (MPS) and fibromyalgia (FMS) there appears to be an alteration or a problem with the pain threshold, perceived versus actual pain. In other words, there appears to be a difference between pain reported and the actual amount of painful stimuli. MPS appears to be a problem of pain perception and expression. Some of the symptoms associated with myofascial pain syndrome include increased muscle soreness and tenderness, particularly in certain and very specific areas (muscles of the upper back, trapezius). Interestingly, as with fibromyalgia, MPS is found more frequently in women than men, the reason for this is unknown. In addition to chronic and more localized pain expression, the syndrome is also known to be associated with sleep disturbances and fatigue. The pain associated with this condition also appears to persist, and often worsen, over time. While pain appears to be specific and localized, it is also chronic and may express itself as headaches, neck pain, pelvic or hip pain, jaw pain, and even arm and leg pain. The leg pain may be mistaken for or diagnosed as sciatica. Which is a mistake or misdiagnosis, sciatica is a symptom, not a diagnosis!</p>
<p>The pain associated with MPS is generally expressed as an aching, deep, almost throbbing pain. Often, the pain in the lower back or hips is described as an aching or throbbing pain. As noted above, the pain often worsens and persists longer than expected, given the diagnosis and the underlying cause or suspected cause. The pain is also expressed as a stiffness in the muscles, and the joints adjacent to the affected muscle. The painful contracture or knot is often expressed as an area of stiffness or tension. The affected area feels very much like a tight spot or and knot and may be sensitive to the touch. </p>
<p>Factors that may be instrumental in bringing on myofascial pain syndrome may include muscle injury, continued stress, both psychological and physical stress to a localized muscle or muscle area, age (MPS is more likely to be diagnosed in middle age women but is also diagnosed in men), a sedentary lifestyle and/or inactivity may bring on pain at certain trigger points in the muscle, and finally, anxiety and stress. Individuals under a great deal of stress often express pain at various trigger points, perhaps due to muscle tension as a result of stress. Some researchers have suggested that the clenching or tightening of muscles associated with stress is a factor.</p>
<p>If pain persists or worsens, or seems to have no real reason for being there or appears to be localized, as with trigger points or knots, then a medical specialist should be consulted. Complications of myofascial pain syndrome may include muscle weakness, particularly due to inactivity as the pain sufferer is on able or unwilling to tax the painful muscle area. Additionally, as noted above, sleep may be a problem, as it is often difficult to get relief from the pain long enough to fall a sleep. Lack of sleep or sleep disturbances may be one of the reasons why chronic fatigue also seems to be related to this condition. Finally, it has been suggested that myofascial pain syndrome may evolve into fibromyalgia in some patients. While MPS is localized and unilateral in its pain expression, fibromyalgia is widespread and chronic, and it is thought myofascial pain syndrome may play a role in this condition.</p>
<p>Myofascial pain syndrome treatment generally includes some sort of trigger point injection and/or oral medications, as well as physical therapy, exercise, stretching, and massage. Once trigger points are identified, the medical practitioner may use an injection strategy called &#8220;needling&#8221; to localize pain at various trigger points. Stretching is generally done to ease the pain at the affected muscle trigger point by gently stretching the area. At times, medical practitioners employ a freezing lotion, spray or solution to numb the affected area or trigger point while treating it. Gentle massage also appears to be effective in some instances, although trigger point sensitivity is a problem. Finally, medications are often used, to include NSAIDs and depression medications, particularly tricyclic antidepressants. Depressants seem to help with chronic pain symptoms and with sleep disturbances, thus reducing stress.</p>
<p>Ultimately, taking care of your self, relieving or alleviating stress, combined with a strategy of exercise, relaxation, and a healthy diet has been shown to be effective. Taking care of one&#8217;s self may go a long way towards effectively dealing with myofascial pain syndrome. Exercise, particularly a program that allows for gentle stretching and controlled movement, is effective. Walking has also been shown to alleviate tension, improve muscle tone, and reduce over all pain sensitivity in many patients. If the myofascial pain syndrome patient is tense, anxious, depressed, and/or stressed more pain may be experienced, particularly neck pain, back pain, hip pain, and sciatica or sciatica-like pain. Meditation, social interaction, either in person or online, writing, journaling, acupuncture, and, in some instances hypnosis, have all been demonstrated to alleviate stress and reduce pain levels. Finally, take care of yourself! Taking care of your body, eat the right kinds of foods, as in a healthy diet full of vegetables and fruit, combined with enough sleep, will help the MPS sufferer cope with the chronic pain, fatigue, and stiffness associated with myofascial pain syndrome.</p>

			<content:encoded><![CDATA[<p></p><p><strong>Chronic Treatment-Resistant Neck Pain, Back Pain, and Sciatica: Psychosomatic Back Pain&#8230;It&#8217;s Not &#8220;All in the Head!&#8221; </strong></p>
<p>Science has demonstrated that there is an incredibly powerful link between the mind and the body. In many instances, the underlying cause of neck pain, back pain and sciatica, also referred to as the back pain complex, may be psychological. Many times, when an individual is in pain and told that the pain may be psychological or psychosomatic, the patient immediately becomes defensive, often stating, “It is not in my mind, the pain is real!” The fact is, the pain is indeed very real, regardless of whether the pain is labeled as psychosomatic or not. When dealing with treatment-resistant, chronic pain the mind/body link must be explored, so it can be ruled out. In this article, we will discuss psychological pain, psychosomatic pain, and chronic psychological back pain. While discussed in a previous article, we will also address, once again but briefly, the link between chronic back pain and depression. Chronic pain, in any form, can be the most insidious, debilitating, and disabling of any medical complaint. In order to fully treat the condition, underlying causality must be explored and diagnosed thoroughly. It is true that most cases of neck pain, back pain, and sciatica are or have their basis in an actual pathology or other physiological condition. However, psychological back pain is quite common and in fact is one of the primary causes of treatment-resistant chronic pain.</p>
<p><strong>&#8220;I&#8217;m Not Crazy!&#8221;</strong></p>
<p>Unfortunately, when an illness is labeled psychosomatic there is often an immediate stigma or negative perception associated with it. Very often, when an individual suffering from neck pain, back pain and/or sciatica hears the term psychosomatic, they immediately respond defensively. When a condition is labeled as psychosomatic the connotation attached is that the condition is “all in the mind” or somehow “imaginary.” There is even a mental illness stigma attached, with the patient protesting, “I am not crazy, this pain is real!” Even people close, at times members of the individual’s own family, will say unkind things like, “I knew he was faking it” or “I knew she was just trying to get attention, I felt sorry for her!” For these reasons and others, the resistance to a psychosomatic diagnosis is considerable, often socially and culturally based.</p>
<p><strong>Stress, Back Pain, and Depression</strong></p>
<p>As discussed in a previous article, there is a powerful link between stress and psychosomatic illness, in this case neck pain, back pain, and/or sciatica; or treatment-resistant chronic back pain. Additionally, the link between depression and psychosomatic illness, particularly as we age, is often quite powerful. Individuals suffering from psychosomatic illness may experience incredible pain or other physical manifestations or symptoms, with no physical diagnosis. Paradoxically, there are a number of physical conditions, such as brain injuries and vitamin deficiencies just to name two, which may exhibit profound psychological symptoms. However, it is often the case that many individuals exhibiting treatment-resistant chronic pain, with no underlying medical diagnosis, may have a psychological or psychosomatic condition.</p>
<p><strong>The Pain is Real!</strong></p>
<p>Contrary to commonly held beliefs, pain that is the result of a psychosomatic condition, of an emotional or psychological process, is actually quite real. Psychosomatic pain, in this case treatment-resistant chronic back pain, is not imaginary, it is not “in our heads,” it is not the result of a delusional or a sick mind, and it is not the consequence of mental illness. The fact is that anyone suffering from tension headaches, ulcers, colitis, tension backaches, and a myriad of other stress-related illnesses has a psychosomatic condition. Clearly, all of these individuals are not “crazy.” I suppose the best way for an individual to understand just how real psychosomatic pain can be is to have them think back to their last tension headache and ask a single, quite simple question: “Was the pain real?”</p>
<p><strong>Psychological, Psychosomatic, Stress-Induced Back Pain</strong></p>
<p>Psychological back pain is simply another name for psychosomatic back pain and is sometimes referred to as stress-induced back pain. Once again, pain associated with this condition is indeed very real. While the pain may ultimately be linked to the mind/body link, the physical expression of pain, whether the pain is neck pain, back pain or sciatica, may be debilitating and disabling. Consequently, dealing with psychosomatic, treatment-resistance chronic pain is often difficult, if not close to impossible, without effectively dealing with the underlying condition. As a noted evolutionary scientist once pointed out, psychosomatic pain may be considered an evolutionary anomaly but it is completely human and utterly normal. As our system, and that includes our mind, becomes overloaded, it seeks to alleviate or eliminate the problem as quickly and efficiently as possible. Unfortunately for the patient, it does this by shifting focus away from the actual cause, the real danger, expressing it in a very uncomfortable, often intense fashion; and, in this case through chronic treatment-resistant back pain. </p>
<p><strong>Failures of Modern Medicine</strong></p>
<p>Psychosomatic pain is the most frequently diagnosed and least understood form of both acute and chronic back pain. Significantly, most treatment-resistant chronic back pain sufferers, those with unresolved back pain, meaning that there is no clear diagnosis, by definition suffer from an underlying and misdiagnosed psychological ailment or condition. While medical schools are beginning to address this very significant medical issue, psychosomatic illness has long been relegated to the back burner of medical education. Consequently, medical practitioners are simply not prepared for or able to understand, much less treat, this condition. As a result, many individuals are misdiagnosed with this “scapegoat condition,” leaving individuals with very real physiological, structural, and medical conditions undiagnosed.</p>
<p><strong>Impact and Consequences</strong></p>
<p>Unfortunately, while solutions are available, they are still relatively unknown. As the medical community begins to educate itself, so they can be better prepared to deal with this condition, we are beginning to see the first steps towards a holistic approach to this problem. Psychosomatic medicine has been talked about for centuries, even Sigmund Freud was fascinated by it in his time, yet it is only in the last few years that we see a real movement to deal with this ongoing epidemic. Treatment-resistance chronic pain sufferers, particularly those individuals with an extended history of unresolved and undiagnosed pain, are most likely to exhibit a myriad of medical complaints to include ulcers, colitis, and depression, just to name a few. The combination of unresolved neck pain, back pain and/or sciatica and depression leads to profound consequences for the individual, the family, the community, and even the state. The impact of this poorly understood and often misdiagnosed condition or set of conditions ripples throughout the system with consequences far beyond those of a medical nature.</p>
<p><strong>Bad Back Treatment Strategies</strong></p>
<p>Ultimately, and until such time as the underlying condition has been diagnosed, treated, and eliminated, symptoms must be recognized and alleviated. For this reason, it is advisable for the individual to begin a wide-ranging, individualized, and medically supervised program to deal with both symptoms expressed and causality yet undetermined. Exercise, in and of itself, will have a remarkable and immediate impact on most individuals. Not only will an exercise program, properly administered and supervised, do wonders to alleviate stress, it will also serve to improve self-esteem and self-confidence. With exercise, particularly until the musculature adapts and adjusts to the new routine, certain other measures will need to be taken. The additional treatment strategies to be initiated, along with the exercise program, should include a stretching program, 2 to 3 times a day (see: “Simple and Easy Program of Exercise for Sciatica Relief… Part I &#038; Part II”). Self-education, both individually and in a group setting, will also enhance understanding, self-knowledge, and self-esteem. Most importantly, either individual or group therapy should be initiated in order to comes to grips with the underlying issues contributing to the unresolved and/or misdiagnosed psychosomatic pain condition. The concert of bad back strategies will do wonders to alleviate symptoms, ultimately leading to a resolution when used along side of a solid therapy program to understand underlying stressors which resulted in the back pain complex of neck pain, back pain, and sciatica.</p>

			<content:encoded><![CDATA[<p></p><p><strong>Chronic Back Pain and Depression Together Create a Black Hole in the Life of the Individual Suffering from Neck Pain, Back Pain, and/or Sciatica</strong></p>
<p>Chronic back pain is a condition that affects millions of Americans every year. Chronic pain of any kind leads to depression in as high as 85% of all individuals diagnosed with the disorder. Chronic back pain may result from any number of factors and is in fact one of the least understood conditions affecting millions of back pain and sciatica sufferers worldwide. The often insidious and frequently misunderstood complex of chronic back pain and depression leads to its own set of conditions and consequences. The combination of depression and chronic pain often leads to total disability, with little hope of a real solution or cure.</p>
<p><strong>Two Categories of Pain</strong></p>
<p>Back pain, in fact pain in general, often falls into one of two categories. The two types of pain are nociceptive pain and neuropathy or neuropathic pain. Nociceptive pain is sensed by what are called nociceptor sensory fibers. Neuropathic pain or neuropathy is a term used to describe damage to a nerve or nerve tissue. With nociceptive pain, messages are sent to the brain signaling an injury to the skin, muscles, connective tissue such as ligaments and tendons, bones and joints or other vital organs. Nociceptive pain may be described in terms of trauma or a specific injury that often heals with time and treatment. Examples of this type of pain include the pain after spine surgery, the pain due to a fall or an accident, stubbing your toe, and arthritis pain, just to name a few. Neuropathy or neuropathic pain is generally a deep sensation, whether aching, throbbing or soreness. Neuropathy is generally associated with back pain and sciatica but may also indicate damage in other areas, such as pain in the neck extending into the shoulders, arms, hands, and fingers. It is believed that in cases where the nociceptive pain is prolonged, with no clear resolution or outcome, it may evolve or progress into neuropathic pain. It is not uncommon for a patient to have a constellation of conditions in which both categories, both classifications of pain are present.</p>
<p><strong>Acute and Chronic Pain: Understanding the Differences</strong></p>
<p>Within the context of nociceptive pain and neuropathy, there are gradations of pain ranging from mild to acute and from short term or abrupt manifestations to a chronic, long term state. While pain is a subjective state and classification next to impossible, we will define pain as falling into one of two basic expressions, acute and chronic. Acute pain and chronic pain are very different, not only in terms of the actual sensation or expression, but in terms what the sensation or sensations are &#8220;telling&#8221; us, as well. Acute pain generally reflects the degree of damage at a specific location on or in the body. In cases of acute pain, there is a positive correlation, a relationship, between the sensation and the amount of actual damage. As a result, pain is considered a protective mechanism, an adaptive response allowing us to remove the cause or cease the behavior, thus interrupting the pain and minimizing the damage. Thus, acute pain is an expression of nociceptive pain. Chronic pain, on the other hand, does not send the same message acute pain does. Nor is chronic pain protective or adaptive, it serves no real biological function either. In fact, you could almost say that the signal is a mistake. The reason? Chronic pain, or neuropathic pain, continues to send impulses to the brain long after the event is over and there is no longer tissue damage to report.</p>
<p><strong>Chronic Pain and Depression</strong></p>
<p>It is chronic back pain, with no clear causality, pathology or otherwise, that is so difficult to treat and leads so many of its sufferers into depression, debilitation, and disability. Depression and chronic back pain are inextricably linked and treatment is difficult, if not impossible. The typical chronic pain sufferer with depression also experiences a loss of appetite, insomnia, irritability, anxiety, and a myriad of other mental and physical maladies, all linked back to chronic back pain. Unfortunately, it is at this point the pain management practitioner usually steps in and medicates the patient with an amazing array of narcotics, exacerbating the situation further. Now we have an individual suffering from chronic back pain and depression, probably disabled and unable to work; and, if he or she does work, they have been completely marginalized by the stigma and their inability to function at optimal levels. The addition of narcotics serves to fully debilitate the chronic pain sufferer, usually addicting them to pain medication in the process. The spiral continues downward into worsening depression, hopelessness, loss of identity, loss of self-esteem, and, very possibly, loss of everything around them that was ever important to them.</p>
<p><strong>The Failures in and of Pain Management</strong></p>
<p>Pretty grim, huh? Well, it&#8217;s not without hope and certainly not without resolution. However, it takes a concerted effort including, at times, treatment to get off the pain medication. But it can be done! More and more chronic pain sufferers every day are seeking alternative &#8220;bad back&#8221; or chronic back pain treatment strategies. Many people suffering from the devastating and deadly complex of chronic pain and depression, addicted to narcotics, are starting to understand that traditional pain management, with its &#8220;let&#8217;s thrown medication at it&#8221; mentality is a black hole from which, if not fully extricated from in time, will completely suck the life out of the individual at risk, literally and figuratively.</p>
<p><strong>The Alternative for the Chronic Back Pain Sufferer: Hope for thee Future</strong></p>
<p>The program? Exercise, get off the medication, education, and re-entry into your life. Sounds simple but it&#8217;s not. The chronic back pain may continue for quite a while and medication may be required to alleviate the symptoms of depression. However, if an individual really wants help, it is available. The result may be a new life, one far from the devastating reality of the old one. Exercise, get off the meds, education, and re-entry. Stare with a good, individualized exercise program, get treatment if necessary to get off the pain killers, go back to school or begin to educate yourself, it does wonders for your self-esteem, and re-enter your life, become involved with your family again, with friends, with your dog if you have one. And then? Take life as it comes, one day at a time, as they say. The results will be worth it. The journey? </p>
