Back Pain, Sciatica, & Surgery, What You Need To Know Before You Undergo Surgery: From The Bad Back Guy’s Personal Experience

In 1970, I was a normal kid, involved in everything and afraid of nothing. I had never experienced back pain in my life and couldn’t even spell sciatica! Bad back? A bad back was something old people had and complained about, not me. In other words, I was the typical fifteen or sixteen year old kid. It was at a wrestling match, before-hand actually, when my world changed forever. Prior to a wrestling match, as was always the case, our coach would crack our backs. Getting your back cracked was kind of a ritual and involved getting back to back with the coach, hooking arms, and then being hoisted upwards while hyperextending the spine. Yes, I can hear the groans from here, but it worked…and it was ritual. You see, wrestlers, like most athletes, are really big on habit, ritual, and tradition. I once knew a wrestler who wore the same socks, never washed, for an entire season. Of course, we stayed away from him after a while, particularly towards the end of the season. So, we had our backs cracked.

Ritual! Tradition! Habit! Superstition!

Well, the season progressed, I’d felt a twinge that night but nothing to get upset about, just a twinge, certainly no back pain or sciatica. Up until then, I had only lost once, in the ninth grade, to a human wrecking machine by the name of Scott Clifford. Scott had been wrestling since birth, or so it seemed, particularly on that day! But since then, never…not once. A funny thing happened on my way to sixteen, I started to lose, and lose big. After a while, I was hearing phrases like “flash in the pan” and ” glory boy,” and “it went to his head.” Kids and high school coaches, as we all know, can be very cruel, especially when the “king” is dethroned. And I was summarily dethroned and dismissed by the end of the season.

Anyway, I went on to track, I was never so happy to start running again in my life, all fifteen and a half years of it. Well, I had a horrible time! I went from holding school records to not being able to get around the track. Kids who had never come close to beating me were blowing by me. It was a terrible season. Halfway through, disgusted and completely disillusioned, I went to see the family doctor. I described my symptons, minimizing and denying of course. I told him about losing in wrestling and then in track, I complained of about the pulled muscle in the back of my leg. I had no idea what sciatica was…but he knew. By that time, I did know what back pain was and, in a moment, I was going to be educated about my pulled muscle.

A Pulled Muscle: Back Pain and Sciatica

The Doc asked me if I had injured myself, felt anything pull in my back? The rest is history! A light went on, I told the doctor about the back cracking incident, told him about the pain, I came clean and admitted it all. What a relief! I had completely removed the back cracking from my consciousness, never giving it another thought, until that moment, but in an instant it was all clear. It was also clear to Dr. Rush and he immediately sent me to an orthopedic surgeon, the next day. I learned all about back pain, sciatica, and leaned new words like spondylolysis, spondylolisthesis, and spinal fusion. On June 12, 1971, I learned all about real back pain. I woke up in the recovery room at Fairview Hospital, Cleveland, Ohio…it wouldn’t be the last time.

Fast forward and it’s twenty-nine years later, the year 2000…Y2K! I am in a whole new world of back and sciatic nerve pain, sciatica, by now. In fact, I am on so much pain medication, have had so many back surgeries, many doctors wouldn’t even see me. The ones who would see me said things like arachnoiditis, chronic pain syndrome, failed back, and a whole array of terms meaning everything and nothing. By 2000, I had undergone 10 major surgeries on my spine, I was to suffer through 4
more. That’s right, fourteen major “procedures” on my lower thoracic, lumar, and sacral regions by the age of fifty.

Little did I know it at the time, and every doctor I saw missed it, I had advanced, chronic osteomyelytis. Osteomyelitis may present with no symptoms at all or may have signs and symptoms so hard to separate from other conditions that doctors miss the diagnosis entirely. Osteomyelitis of the back (vertebrae/spine) or hips
(pelvis) may have few overt signs and symptoms, no back pain, and no sciatica. Osteomyelitis that occurs after surgery or a fracture or deep wound may present with
swelling and pain but you may attribute that to the traumatic event, not an infection. And it was the infection, in 2000, probably there since 1994, that almost took
my life. Had it not been for an amazing doctor at a noted Cleveland hospital, the other one, the one attached to the noted eastside university, the really famous hospital missed it too, I wouldn’t be writing this today.

So, five surgeries and nine years later, yes, that’s a total of fifteen, I am finally getting my life back. As far along as a year ago, it was still touch and go, major complications as a result of the osteomyelitis, combined with other health issues, almost did the job that we avoided back in 2000. Yes, that’s right! The math doesn’t work. That’s because I also crushed my shoulder in 2007, a consequence of weak bones due to the infection and other problems. Now, the only metal left in me is in my shoulder and that makes fifteen!

Now, should you have surgery number one? No one can answer that for you, that’s something you must face, and answer for, yourself. Back pain and sciatica can be
terrible and there comes a time where anything will do, any solution seems OK…as long as they get rid of the pain. However, before I opened Pandora’s Box again, for the first time, I would do everything humanly possible, exhaust every option, before having back surgery. Don’t open Pandora’s Box if you can avoid it! You will never be the same and, of the half a million or so who undergo surgery every year, only 30-35% have a total and complete recovery, 20% never recover and are worse off. Not the kind of odds I’d go to Vegas with, if you know what I mean. Back pain and sciatica can be terrible, I’ve been there, this isn’t just a topic I drew out of a hat. My doctoral research was on the spine and sacroiliac (hip joint, the ilium meets the sacrum or hips to tail bone), so I know the situation, inside out you might say.

Back pain and sciatica? Yes, they are a reality after so many surgeries, they really can’t be avoided. Are they as bad as they were? No, absolutely not. A program of
exercise, to regain muscle tone and lose weight; ice, with the use of an ice-compression brace, to reduce inflammation; sleeping with five pillows (yes, five!) or, when I need to, in a recliner fully reclined and a pillow under the knees, try it, it really works wonders on the nights when nothing else works; and finally, and a good pair of New Balance running shoes. Yes, New Balance and, if you must wear a harder shoe or boot, orthotics for cushioning. No, another kind of running shoe isn’t as good, I’ve tried them all over the years and New Balance, for the heel cushioning, are the absolute best. And no, you can’t get the relief or the cushioning from the insoles of Doctor S, it’s not the same! Am I “spamming” you? No! Go anywhere you want buy anything you wish. But if you want relief, these are the ultimate in bad back strategies, strategies developed over three decades, give or take, for dealing effectively, and not so effectively, with back pain and sciatica.

How to Stop Back Pain and Sciatica Without Surgery: The Bad Back Guy

Back pain and sciatica are treatable! Back pain and sciatica sufferers are not doomed to the surgeon’s blade. A bad back can be dealt with effectively, without surgical intervention, if proper bad back strategies are implemented early and followed consistently.

Have you ever gotten out of bed and felt that nagging feeling in your back, a kind of stiffness that doesn’t seem to want to go away with normal movement? Have you ever started to bend over and felt that twinge in the lower back, perhaps radiating down the leg? Have you ever bent over to pick up a child, a pet, even a package and felt a blast of pain? Has your back pain and sciatica ever gotten so bad you had to stop what you were doing and rest?

If you have answered yes to any of the questions above, you are in good company. Unfortunately, back pain and sciatica are a pain complex suffered my literally millions of Americans every year. In fact, almost half a million people are operated on every year for what most people refer to as a bad back. Severe and chronic pain may be caused by many things but is often the result of the stress, strain, and trauma we impose on our muscles, connective tissue (tendons and ligaments), discs, and even the vertebrae themselves. While it is true that many back injuries are the result of significant trauma and disease, the majority of back pain and sciatica results from normal, day-to-day activities. The activities resulting in back strain and injury may be as mundane as working around the garden, sweeping the driveway or raking leaves, shoveling snow (always a good one), lifting laundry or grocery bags, household chores, and picking up children or grandchildren.

Often, simple adjustments to movement and behavior, properly and consistently implemented, go a long way towards preventing or at least minimizing back strain and its consequences. A few bad back strategies in particular are quite effective in minimizing the consequences of back stress and strain: first and foremost, be aware of body position while engaged in activities that may impact the back, particularly the lower back; next, maintain good posture through exercise, conscious thought, and eventually, habit; third, make sure to stretch often, usually as part of a total exercise program; along with the third bad back strategy, integrate a consistent, individualized, and professionally supervised exercise program into your life; next, make sure to always know your limits; and, always listen to that still, small voice in your head, if it says you’re pushing it, you probably are; finally, lose weight. Significantly, added weight may have more of a negative impact on back health that any single factor. If you are carrying extra weight, you are placing incredible stress and strain on you back. The combination or all of these strategies, tied to an intelligent weight loss and exercise program, will do wonders, often alleviating, then eliminating, back pain and sciatica

Ultimately, a medical consultation may be the wisest strategy, particularly if your back pain and sciatica continue for more than a few days. Do not wait for it to worsen! If you have engaged in an exercise program and are losing weight, you should begin to experience relief quickly. You will be amazed at how soon after beginning an intelligent, focused, and individualized exercise program you start feeling real relief. If you do not, see your physician and he or she will recommend a good orthopedic surgeon or neurologist. Don’t wait until you are disabled to get help, it is not necessary and the damage may be irreversible without surgery.

Bad back strategies are easy to implement and, from personal experience, are easily made habits, allowing you to significantly minimize, and ultimate eliminate, your pain. After years of pain and surgeries, weight loss and exercise, along with the other strategies mentioned above, allowed me to re-enter life as a participant and not just as a spectator, miserable, depressed, and disabled. It is not easy and it takes conscious effort but it is doable and the results are amazing! So remember, posture, positioning, lifting, exercise, and weight loss. If you follow these five bad back strategies you will alleviate, and eventually eliminate your back pain and sciatica.

Back Pain & Sciatica Spanning Four Decades: How I Finally Beat Back & Leg Pain Once & For All!

Part I

Hello!

My name is John P. J. Zajaros, Sr. but my friends either call me Jack or Coach Z. I think we are going to be friends, at least I hope so. You see, I believe anyone who suffers with back pain and sciatic nerve pain, also called sciatica, as we have not only have something in common, we have a bond, a very real connection. To my way of thinking, anyone who suffers with back pain and/or sciatica, and it’s pretty hard to have one without the other to one degree or another, knows pain and suffering as no other acute-chronic pain sufferer can. Over the next few weeks, months, and even years, I hope, we can come together as a community and share our experiences, hopes, dreams, and stories so others may benefit and, hopefully, pass that benefit on to others. That’s my hope anyway. 

So, here goes:

I was the All-American high school kid, just 15 years old, living my life, competing in everything, and president of everything else. I’d just finished the last requirements to win the Eagle Scout Award and was in the middle of the wrestling season, at Bay High School, when my life changed forever. I was getting warmed up before my match, which was next, and my coach, Don Chadwick, asked if I wanted my back cracked…something he routinely did to loosen us up and get us limber before we went out on the mat. I said, “Sure,” as I always did. So, he got behind me, locked my arms, and pulled me off my feet and hyper-extended my back.

I can hear many of you groaning from here!

Well, you guessed it! I felt a twinge, nothing earth-shaking, and I went out and won my match. I always won…up until then, anyway. As I walked off the mat, my legs buckled a little, nothing major I thought, nothing to worry about. As usual, I’d overdone the weight loss thing that week and was a little unsteady because of it. But there was that twinge! I couldn’t put my finger on it but in the back of my mind all sorts of bells and whistles were going off. But hey! I was 15, indestructible and immortal, what did I have to worry about?

The rest of the season was a disaster!

I couldn’t seem to do anything right. Many times, if someone were on top of me, my back would simply give out and that would be that. Losing was something new and very different, I hated it! But I still had no idea what was happening to me, I just thought there was something wrong with me. You know the me I am referring to, the me as a human being, the psyche me, that intangible me. It had to be me, that me we all refer to when we ask the age old question, “What is wrong with me!” But what part of me? I had no idea. I was never so happy to see a season end, and I loved wrestling! I even started running with the track team early, I just wanted it all over.

But it was just beginning!

Track started with a bang, starter gun and all! My freshman year had been amazing, records, ribbons, and everyone loved Jack! My sophomore season was going to be even better, varsity as a sophomore was a very big deal at Bay High back then. I’d accomplished it in wrestling, if only for a short time, and now I was primmed for track. I loved the attention! I just knew I was on to big things, “scholarship” was being whispered by the coaches and I ate it up. But something funny happened on my way to great expectations, I started getting beat! I don’t mean by Bobby Pressler, my biggest competition for the fastest kid in the school honors, I mean by guys I blew by only weeks before. I can’t explain it, even today, I just couldn’t move my legs any faster. You see, I knew I could go faster, my brain knew I could go faster…but my legs? Well, they had an entirely different idea about how fast I was going to go…and they had the final say!

Humiliation!

Talk about a bitter pill! The coaches began to shake their heads, “Flash in the pan,” they whispered. My classmates just didn’t know what to say. And my teammates? Well, they were stepping right in. After all, the king was dead, long live the king! The rule in athletics, even at the professional level, get hurt or injured and lose your spot. Mine, my spot? Well, it had history written all over it! One month into the season? Yup! I quit. And quitting was something I had never done before. It was a very bitter pill and I had to swallow it whole!

Back Pain and Sciatica? What is Sciatica?

By the time I made it to the doctor’s office I had stopped denying the pain in my back. By my first visit it was so real I was in agony all the time. During the exam, as he was poking and prodding, he began with a series of questions. Doc Rush, I had always called him Doc, asked if I had pain in my legs. I said, “Yes Doc, my right one but I just pulled a muscle.” Doc nodded and smiled a bit. But it wasn’t a good smile, it was one of those “Ah Ha!” smiles, like he knew something I didn’t. I never liked that smile from doctors much, still don’t! It was during that visit I heard the word sciatica for the first time, it wouldn’t be the last!

Back Pain & Sciatica: Now Add Spondylolysis & Spondylolisthesis

Well, it was official, I was all messed up! My next stop was an orthopedic surgeon, the first of a parade. I went through so many x-rays and tests I was beginning to feel like a guinea pig in some mad scientists experiment. Once the tests were back, and I was seated once again on the cold plastic table in nothing but a cloth gown (they hadn’t moved to paper quite yet), I was scared. By this time I didn’t know what to think, I just knew I wanted out of there just about as bad as I wanted the pain to stop…about! When Dr. John D. Kenney walked into the room, I knew the game was over. He nodded, they do that a lot, and started throwing around terms I’d never heard before. Soon however, he got to one I didn’t know…surgery!

It went something like this: back pain & sciatica + spondylolysis & sponylolysthesis = surgery!

Pain Leaves No Options

I knew I was sunk! I also knew I had no choice, I was “a-hurtin for certin” by then. The funny, or not-so-funny, thing about pain is that when you experience it, in all its worst manifestations, all you want is for it to be gone…NOW! back pain and sciatica had taken a healthy, 15 year old All-American young man and reduced him to a limping, hurting child who would agree to anything just to be free of it, if just for a while. So I agreed! I said yes! I said uncle! I said anything and everything I needed to, just to be pain free for a while. Little did I know, I was opening Pandora’s box!

Back Pain + Sciatica + Spondylolysis + Spondylolysthesis + Surgery = Pandora’s Box

Yup! I did it! All of 16 by a couple of months and under the knife for round one. As I’ve said before, I would have done anything, said anything, to be pain free. But to get there? Oh boy, to get there! When I woke up in the recovery room, intubation tube still in my throat, I knew three things with a certainty I’d never experienced before: 1) I wanted to breath and couldn’t…so I wanted breath; 2) I was in more pain than I had ever been in before…so I wanted it to stop; 3) I knew the pain wasn’t going to stop any time soon…so I wanted to die! That was the first time in my life I wanted to die…it wouldn’t be the last. As I think back to that day, in those moments, I knew one more thing with complete and utter certainty, if Hell was on earth, I was there, both feet firmly planted in it…no doubt about it!

Pandora’s Box: “Hindsight is 20/20!” & “If I knew then what I know now!”

Yes I know, everyone says those two things, particularly when they are about to dispense advise…usually about something they have absolutely no idea what they’re talking about! But I have to tell you this, and if you are still reading this, it’s because you know, when you are in pain, when pain has brought you to your knees, you forget everything about what has come before, all the pledges you’ve made to never go through anything like that again, and you do whatever it takes…AGAIN! You reopen Pandora’s Box! Little did I know, as I suffered on that gurney in that very stark and cold recovery room for the first time, that I would reopen Pandora’s Box thirteen more times! Back pain and sciatica can be there own, very special kind of hell, one that makes you forget that other hell you awaken into after every one of the surgeries past.
